Job Title: RNLD Case Manager – Complex Care
Salary: £45,000 – £50,000 (DOE) + Mileage
Hours: 37.5 hours – Monday to Friday
Location: Remote with travel across Guildford and Crawley
Caseload: Paediatric and Adult Complex Care Packages

As a Case Manager, you will be at the forefront of managing a diverse caseload, offering mentorship, and contributing to the organisation's expansion.

As a Case Manager, your core responsibility lies in ensuring that your clients receive optimal treatment and maintain exemplary standards of care within the sanctuary of their homes. Your role involves collaborating with commissioners to design new care packages, carefully selecting staff for each case, and providing unwavering support to both clients and staff members. Moreover, you'll be entrusted with meeting the clinical and social expectations of your clients by cultivating positive relationships, upholding care standards, and employing cutting-edge case management techniques to mitigate hospital admissions. Compliance with CQC regulations and the implementation of best practices are integral components of your role.

Your specific duties include analysing the physical and mental health needs of clients, collaborating with stakeholders to craft personalised care plans, and conducting risk assessments for clinical interventions as required. Providing support to clients and their families, conducting staff supervisions and appraisals, are pivotal aspects of your role.

Hybrid working pattern, balancing remote work with hands-on clinical involvement in Guildford and Crawley. Travel across the area is an integral part of the role, promoting clinical guidance and training. Monthly visits to the Central London headquarters will also be required.

Shift Pattern:
Monday to Friday, with the flexibility to manage your own diary. Occasional evening and weekend work may be necessary based on package demands and training requirements.

– Registered Learning Disability Nurse (RNLD) + Active UK NMC Registration
– Minimum of 2 years post-qualification experience
– Valid UK driving license and access to own vehicle
– Demonstrated proficiency in Case Management within Learning Disabilities, Brain Injury, and Mental Health.
– Proficient in interviewing and effectively matching staff members.
